You have reached the Allina Health Caring for Colleagues Emergency Fund Application homepage. The Allina Health Caring for Colleagues Fund is accessible to employees and former employees who are experiencing financial hardship due to an unavoidable disaster.

APPLICATION GUIDELINES
- Applicant must be an employee of Allina Health, or a former employee impacted by involuntary reduction in force applying within one year from termination date. Termination must not have been for cause.
- Applicant is eligible to receive one grant in the maximum amount of $2,000 in any given 12-month period.
- Applicant is eligible to receive a lifetime maximum of 2 grants.
- Applicant must be experiencing unexpected and severe financial strain as a result of a crisis event.
- Application to be completed by the employee in need. Applications submitted by co-worker’s will not be accepted.
- Your application will be handled with respect and confidentiality, while allowing for appropriate review and evaluation.
§ Grant money will not be paid directly to the employee. Please attach copies (not originals) of current bills or invoices that you are requesting be paid on your behalf.
- Billing statements should include the portion of the page “detach and send in with payment” often referred to as the remit slip or payment coupon. It includes: amount due, account number, and where to mail payment.
Examples of eligible expenses include: Rent/Mortgage, Utility bills, monthly Car payment, Car repairs, etc.
Examples of ineligible expenses include: Insurance premiums, bills not addressed to the employee or residence, cable/satellite tv, credit card debt, expenses incurred while on strike, elective expenses, income tax assessments, property tax, tuition/student loans, etc.

§ Incomplete applications will not be reviewed. All sections of the application must be complete before sent to the Review Committee.

This is a confidential service available to you and your household members. Support could include but is not limited to the following examples: connecting with community resources for help with groceries, meal delivery, utilities, rent/mortgage assistance, connection with mental health support, accessing Allina Health Employee Well-being Benefits and Optum resources/benefits.
